Many women can now receive no cost birth control, Pap tests, STD testing and treatment and more through SelectPlan for Women - a no cost plan that covers family planning health care.

Do I qualify for SelectPlan for Women?

You likely qualify if you...

are between the ages of 18 and 44 years old
are a U.S. citizen or permanent U.S. resident
are a resident of Pennsylvania
have no or limited health insurance
are not pregnant
meet income requirements

How Do I Sign Up for SelectPlan for Women?

You can enroll in SelectPlan for Women at any Adagio Health location or other family planning provider. Click here for a location near your or call 1-866-520-9139. Our skilled and trusted health care professionals will provide answers to your questions.

When you come to your next appointment, please bring the following information with you:

Social Security number
Birth Certificate or U.S. Passport
PA driver's license or state-issued photo ID
Proof of income for all working family member in your household
- A recent pay stub
  or
- Employer's letter stating pay for a typical pay period
or
- Proof of other income, such as child support
or
- If self-employed, a copy of tax returns
Insurance card if you have health insurance

If you don't have all of these documents, please bring in what you do have and our staff can help get your application started.

Is there any cost to me if I enroll in SelectPlan for Women?

No. This is a no cost coverage plan. If you qualify for SelectPlan for Women, there are no co-pays for your visits, no visit limits, and no cost to you for your birth control prescriptions. Services will be provided at no cost to you beginning the day you fill out the application.

What if I don't qualify for SelectPlan for Women?

If you don't qualify for SelectPlan for Women, you may still qualify for confidential, no cost or reduced cost family planning services through Adagio Health.

Adagio Health's medical offices participate in all Medicaid plans, Medicaid
and SelectPlan for Women, and we accept most commercial
insurance plans. For those without insurance, fees for most
medical services (excluding prenatal care) are based on
income and family size. Most medical services are provided at
no cost to those age 17 and under. Breast and cervical cancer
screening may also be available at no cost to those who
qualify.
